

NEED TO MAKE BENEFIT CHANGES...
Unless you have a qualified change in status, you cannot make a change in the benefits you elect until the next open enrollment period. Qualified changes in status include:
Marriage
Divorce
Birth or Adoption
Legal Separation
You have 30 days after a qualifying event to log into Paylocity and make a change to your benefit elections.
How Do I Enroll?

Log in to your Paylocity Account via https://access.paylocity.com/.

Click on the “HR& Payroll” tab and select “Benefits” on the side menu.

Click “Action Needed” in the left panel and then select "Start."

Select “Start Your Enrollment” to begin selecting your benefits. Complete each section for election or declination of Assembly Health benefits.

Review all elections and select “Submit” to complete your enrollment.
What Do I Need To Enroll?

Take time to read the entire benefits guide to educate yourself and family members on your options.

Decide on your benefits with your family, if necessary, prior to starting the enrollment process.

For your elected beneficiaries and covered dependents you will need their Social Security number and date of birth.
